Investing in Your Transition and Your Ongoing Impact

We offer up to $15,000 in funding to support physician transition to practice in Brantford.  This flexible funding can be used to offset relocation expenses, assist with practice setup, and support your involvement in teaching and mentoring future physicians.  

Partner and Family Support
We recognize that your decision to relocate impacts more than just your career. We offer support to help your partner explore local employment opportunities and connect your family with resources such as schools, childcare and community services. 

EMR Education and Onboarding

We will support a smooth transition into your clinic with access to EMR training and onboarding resources.  Whether you are brand new to the system or looking to build confidence, we help ensure you feel comfortable and ready from day one.

Practice Models

Family Health Organizations
FHO

  • 6+ Physicians working as a group
     

  • Physicians enroll patients to their individual practices Regular office hours plus a minimum required after hours block
     

  • Blended capitation model – complement-based remuneration plus bonuses and incentives

Family Health Group
FHG

  • 3 or more physicians practicing together either in the same office or in close proximity
     

  • Physicians enroll patients to their individual practices Regular office hours and 3+ after-hours blocks (3 hours) based on the number within the group
     

  • Fee for service model with a small bonus for enrolling patients

Community Health Centre
CHC

  • CHCs are interdisciplinary teams that serve communities and populations who may have barriers to health services; addressing underlying conditions, such as social determinants of health, diet and literacy.
     

  • Employs multi-disciplinary healthcare professionals such as nurse practitioners, social workers, outreach workers etc.
     

  • Salaried model

Family Health Teams - FHT

Family Health Teams are collaborative primary care organizations that bring together physicians, nurse practitioners, nurses, social workers, pharmacists, dietitians and other healthcare professionals to provide comprehensive team-based care.  FHTs support patients through enhanced access to allied health services while helping providers deliver more connected and coordinated care.
